Julie Lynn and Tim Hayes present the Sunderland City Council’s Integrated Care Intelligence Platform. In this session, they outline the development of Machine Learning (ML) and Artificial Intelligence (AI) data analytics on Sunderland Smart City Platform to drive better social care and health outcomes for the people of Sunderland, and more efficient/cost-effective social care delivery through:
- integration of structured/unstructured data including: social care, health, housing, in-home sensors, open-source data, environmental
- AI/ML predictive event-driven workflow actions and care interventions
- predictive models for proactive individual and population-level care interventions, informing future commissioning arrangements and service redesign.
ICIP will generate new insights from disparate and previously separated data sets to enable/inform:
- cross-agency strategies and service provision in previously unforeseen/unattainable ways
- a single view of the care recipient, streamlining/optimising care provision
- a commercial blueprint and reusable data-driven product, replicable across other social care providers
